    1. To be entered into the selection process for this year’s study you must complete the 2005 InformationWeek 500 qualifying application. You may complete the entire application or choose to be recognized for specific achievements by submitting essays detailing key initiatives conducted by your IT organization in the past 12 months. Organizations that completely fill out the application score consistently higher in the InformationWeek 500.

    2. Only company personnel are forwarded the user I.D. and password needed to access the InformationWeek 500 application. Any personnel not directly connected with the company, such as outside publicists, must submit authorization to receive this information from the company they represent and this authorization must be submitted to the InformationWeek 500 mail box, iw500@cmp.com

    3. All information submitted to the InformationWeek 500 must be signed off by a senior-level company executive.

    Please be assured that the information provided by your company will be used in aggregate only across the full InformationWeek 500 or by the study's 21 industries. Nothing will be attributed to you or to your organization without permission.
