Learning to swim starts early for most
Agree that it's a huge problem, but super majority either have it or they don't, so a bit of advice is like a bothersome mosquito--this lesson starts early. Others do (have it) but are in an enterprise culture that doesn't encourage or incentivize any risk or initiative. As is often the case, what majority perceive as low risk usually isn't--for their own careers and organizations. Risk averse cultures quite literally can't provide value--often translates to a negative value in IT, and so many have become more of a liability than an asset. This includes some CIOs in the largest organizations--including IT giants, who admit publicly to not even taking risk their CEO is requesting!

Always good advice?
Charles, at what career level is it too precarious a proposition for an organization to throw someone into the deep end? CIO? VP level? At some point up the IT career ladder, we want experienced people who've been there, done that, no? Please give us a few examples of positions you or the folks you've interviewed have taken on without having what would normally be considered the requisite level of experience.
