It is good that the public sector is gaining momentum with the health care IT department by working hand in hand with the private sector and making better use of the off shelf technology.  This is making things much more affordable to the customers. Implementing new technology and managing them well hand in hand will cut down the cost. The new technology is what brings about the efficiency cutting down on costs which is the main goal.

Alsion, I think you hit the nail on the head: A lot of what ails government agencies are procurement rules, plus the culture to create systems in house, so as not to lose funding, when the smarter approach is clearly building on the work of others.  That DoD and VA have been at this so long, and still struggle to align their systems effectively is a testament to how hard it is to change IT tactics.

An Eye on Costs
While it's outstanding to see some government organizations push the IT envelope, it's even more exciting to see they're more open to partnering with commercial entitites. There's no reason for government agencies to replicate skills readily available, often at a lower cost, from commercial partners - especially if it leads to the creation of full-time positions for contract needs. Overruns and poor tech capabilities continue to plague some government IT contracts. Obviously, we have as one example. But there was also the VA EHR, the DoD EHR (same link), etc. Not sure if government procurement departments' are doing something wrong, if there's a problem in the bidding process, or where the issue lies, but it's apparent something needs overhauling.
