I have been implementing EHRs in practices for nearly 10 years and have helped 100s of physician nationwide. It is true, you take a hit in the beginning as far as productivity, but I've found that physicians who are willing to take the time to learn it (not on the fly between patients but actually sit down after hours and work test patients and build macros/favorites etc...) will be able to get back to pre-implementation numbers.

It will never be as easy and scribbling stuff on a piece of paper or dictating into a dictaphone -- that is what I tell my physicians right off the bat. The ones who waste their energies bellyaching over click counts and focus on the negative impacts are the ones who suffer the consequences of decreased productivity.

Forget meaningful use monies and penalties for a minute. What about the next time there's a FDA alert or massive drug recall (like Celebrex in 2005). While the paper based offices scramble to collect the charts on each patient who was on the drug and call them to let them know, the electronic offices will run a single report and contact their patients electronically in a few clicks of a mouse.
