I don't think ACOs are going to fly while still voluntary. I haven't seen any numbers on how many ACOs there are out there now, but with mandatory Meaningful Use and other initiative out there like NCQA Medical home, I don't think many practices/providers are going to take on any more than they have to. I see many providers who are scrambling to do what they have to just to avoid the MU penalties in 2013.
