From a similarly aged IT pro with 15 years equally divided between technical and department director roles, it can be summarized in two words - professional network. SMBs want a managing technicians (so if your certifications are no longer current get them to be) and large enterprises want bean counters that also know how to spell IT (more frequent a challenge than some will admit). Either way however, with no professional network to get you in the running, to call it uphill in today's market may be generous.
