Excellent article! I work as a contractor in the federal space and see the "fear factor" continually. It drives costs higher and reduces government effectiveness by slowing down decision making, limiting innovation, and fostering over-engineering to reduce risk. It happens because the penalty of failure is getting dragged before congress and being in the Washington Post. There isnt the balancing factor a commercial firm has - losing money and going out of business by not taking risk and innovating. A great example of government going way out on a limb was the program to land a man on the moon. In that case the President stepped up with a clear objective and stood behind it. This is the model for government execs who aspire to be true leaders and make a difference.

I've seen it also through decades of on and off govt collaboration and consultancy. It is clearly THE greatest impediment followed closely by unions that fight to maintain the status quo and personnel bloat (similar to the FAA of the 1980s) and indifference (instead of focusing on the service or product provided, the focus is personal or project based - survival until the next election).
