You hit the nail on the head... The Human Element. The one element that businesses forget when preparing their Business Continuity and Disaster Recovery plans. During a disaster there are so many other important things that are going on, so it's hard to imagine all the possibilities/outcomes that could occur. Moreover, if people haven't practiced their plan, Forget it! It's like playing chess for the first time, You're destined to Lose.
I know a company, Agility Recovery, that offers testing with their solution and that would help with the employee/implementation aspect of this topic.

Your third point regarding rouge IT really struck a point with me. One of the features in a product we're working on at Neverfail is the ability to identify important systems based on network patterns. If a particular machine or service is receiving a lot of connections, it stands to reason that there is something important about it. These important machines appear larger on a heatmap-type diagram, and they're flagged as "hotter" if they're not adequately protected or documented.
