It's good to hide the reality behind load balancers, but it does sound rather odd not to offer anything beyond layer 4 loadbalancing because as the article says, most web apps need more than that in order to run efficiently and keep session state appropriately. All very well saying to design your apps for the cloud but that does rather ignore the reality of what's needed, and would appear to suggest that migrating existing apps to Azure will be impossible without rearchitecting / rewriting the application?

About time
"Virtually all web applications require a load-balancing mechanism for scale-out and high availability. To address this requirement, Windows Azure provides a built-in load balancing layer in its architecture. In fact, the only way to access virtual machines created in Azure from a client machine on the Internet is to send the data via the built-in load balancer.

About time, I've seen systems where they have loadbal.* to initial gateway the system, but then a user gets logged in and bookmarks server1.* and bypasses the whole thing
