The most practical benefits I've seen have been in the hospital EDs.  Either it's a single integrated system or the hospital is pulling key data from its sponsored ambulatory EHR into the acute care system.  And/or they have provided access and training to ED staff and physicians.  So when a patient presents to the ED, details like active problems, medications, and allergies can be taken into account when making care decisions.  The primary care physician in particular also benefits because of the flow of information back to the clinic upon discharge for more timely and appropriate follow-up care; this is definitely one of the advantages the physician weighs when selecting an EHR and a technology partner.  The patient experience is enhanced at all steps in this process because they don't have to repeat information that they've already provided to someone on their care team and they're less likely to fall through the cracks in terms of follow-up activities.

Payoff from EHR?
Are there examples of hospitals that have seen a real, practical payoff in terms of better care coordination when their affiliated providers are on EHRs? Among other things, I'd think this would be a way to steer providers to particular EHRs that may be easier to interoperate with.
