To your point about process, none of these systems will provide a blicking red light that says "you have a quality problem here." They require engineers, product experts, to look at the output and assess what action to take.

Is GM monitoring even after the fact?
I started reporting on data-analysis of warranty and auto-repair data more than five years ago. The Ford Explorer-Firestone tire rollover problem? Preventable. GM's latest flap with the Cobalt recall? Also preventable, but only if you're systematically looking at the data. The tools are there. The practices must follow.
