"Did not deliver software free of defects"
Find me any contractor or development department that is able to deliver software free of defects....I save you some time, because there is none. Defects happen and the far more important issue is how they are dealth with. I rather see a fix come in within a day after release than wait months until all issues are fixed, many of them may not even come up ever in production. It also begs the question if the requirements were free of defects. I doubt it.

Stockholm Syndrome
I'm in the HVAC controls business and years ago I realized that our field people sometimes suffer from "Stockholm Syndrome" where they are out with the customer so much that when something is wrong they begin to instinctively blame our company without researching the facts. They can also get "Santa Clause Syndrome" where they begin to give away services which are not in the proposal.

When IT vendor & client are at odds…
Problems with contract details and joint responsibilities are common between IT vendors and clients.  Most of the time it's just the vendor or the customer has not done its diligence.  However, I am surprised to learn the affinity between a client's own IT staff and the vendor.  Also, the idea that decision delays are more costly than a wrong decision that can be corrected if subsequence correct decisions are made promptly is also unexpected, but make sense once thought through.  Thanks for the piece.
